Prisma Health is implementing a phased approach to digital transformation. The journey began with an assessment of the digital maturity of the enterprise and benchmarking its digital initiatives against industry peers. The assessment also identified gaps and opportunity areas relative to peer group health systems across a range of possible digital initiatives for consideration. Twelve different opportunity areas were identified and then categorized by a ranking system that was used to determine near and medium-term priorities for a phased implementation.

A “Virtual Hub” model was created as a centralized enabling function to drive a number of virtual care programs, including remote monitoring and home health, virtual primary care and specialty care, acute and post-acute care, and more. The road mapping exercise was conducted over a period of three months. An extensive group of senior Prisma Health stakeholders was championed and led by the chief data officer and a team of digital health practitioners.
